“DBS in treatment of movement disorders” educational course.
“Deep brain stimulation in treatment of movement disorders” educational course was held at the premises of the Federal Neurosurgical Center (Novosibirsk) on November 19-20, 2015. The following specialists took part in the event:
· Aleksey A.Tomskiy, MD, Head of functional neurosurgery group of the Academician N.N.Burdenko Neurosurgical Institute (Moscow);
· Viktor G.Simonov, «Medtronic» company;
· Anna V.Rosinskaya, Head of extra pyramid disorders Center (Vladivostok);
· Denis Y.Karpov, MD, Head of neurological division of Municipal hospital #5, Barnaul;
· Vladislav G.Abramov, Head of clinical trials organization, elaboration and implementation of innovative neurological technologies division, the Center of innovative neurology affiliated with the Federal Siberian Research and Clinical Center, Krasnoyarsk;
· Aleksandr B.Dmitriev, MD, Head of functional neurosurgical division, Novosibirsk;
· Natalia P.Denisova, neurosurgeon of functional neurosurgical division of the Federal Neurosurgical Center (Novosibirsk);
· Dmitry Y.Rogov, neurosurgeon of functional neurosurgical division of the Federal Neurosurgical Center (Novosibirsk);
· Elena A.Khabarova, neurologist of functional neurosurgical division of the Federal Neurosurgical Center (Novosibirsk).
The goal of the course was to discuss the role and perspectives of deep brain stimulation (DBS) in treatment of extra pyramid diseases.
7 reports were given during the first date of the course.
The first reporter was Viktor G.Simonov who told about MEDTRONIC products for DBS. He presented the whole range of electrodes and generators of the company made for different types of deep brain stimulation.
Aleksey A.Tomskiy presented the experience of the Academician N.N.Burdenko Neurosurgical Institute in his report “Neurosurgical treatment of movement disorders”. He emphasized the review of modern methods of surgical correction of extra pyramid disorders; he presented his own treatment results. Dr.Tomskiy also told about DBS perspectives in other spheres of neurosurgery.
Anna V.Rosinskaya presented her report on “Organization of medical care delivery to patients with movement disorders through the example of Primorsk territory”. A special character of the report was detailed review of the main types of extra pyramid diseases and also basic parameters of neurostimulation being used for their correction.
After a short break a series of reports was continued by the report of Elena A.Khabarova on “Modern criteria of patients’ selection with extra pyramid pathology for surgical treatment” where Dr.Khabarova presented not only indications for DBS but also criteria of patients’ exclusion from DBS list.
Aleksandr B.Dmitriev was the next with his report on application of DBS for dystonia treatment.
Natalia P.Denisova told about DBS in treatment of essential tremor.
The last report of the first day of the course was “DBS in treatment of Tourette syndrome” by Aleksey A.Tomskiy. He told about perspectives of deep brain stimulation in patients with this rare pathology.
After oral reports there was a real-time translation of injection method in severe forms of muscle dystonia performed by injector, Denis Y.Karpov.
There were 3 reports given during the second day of the course and also 1 surgical DBS show-case.
Dmitry Y.Rogov told about perspective of DBS in treatment of epilepsy. His report contained information about application of DBS in patients with drug-resistant epilepsy.
Anna V.Rosinskaya told about post-operational management of patients with installed DBS system. There was information not only post-operational programming of DBS generators but also about necessity of close cooperation of a neurologist and a neurosurgeon in post-operational period.
Vladislav G.Abramov presented information about experience of organization of medical aid for patients with movement disorders and implementation of new technologies into clinical practice in the Center of innovative neurology, extra pyramid diseases and botulin therapy of Krasnoayrsk.
The conference finished with real-time translation of a surgical case: implantation of DBS system to patient with idiopathic Parkinson’s disease. The doctors of functional division of the Federal neurosurgical center performed this procedure and commented on peculiarities of this procedure: these rare surgical procedures are performed by neurosurgeons and neurologists in conditions of direct contact with a patient, i.e. without general anesthesia. It is necessary to estimate preciseness of electrodes implantation into subcortical brain structures in real time. After implantation of electrodes the doctors stimulate subcortical centers of a patient and immediately get medical effect by way of the main display of Parkinson’s disease regress. At the same time moderator, Aleksey A.Tomskiy, a very experienced neurosurgeon, commented on all the stages of the surgical procedure from the conference-hall and answered the questions of the audience.
All interested specialists were welcome to visit the event. Such events become traditional for our Center because deep drain stimulation is so-called “golden standard” in treatment of patients with extra pyramid disorders all over the world and the staff of the Federal Neurosurgical Center tries to fit into world tendencies.
